Start the principle of 'working together' by ensuring that Primary and Secondary care with the NHS is not in separate silos? As an example, all citizens have a unique identifier (their NHS #) - but there is no access to an online NHS record for the individual citizen which shows the complete personal record and data 'all in one location', to which the social care data can be added.
